Membership Terms
Three-month minimum, paid upfront. Cancel with 30 days’ written notice. Early termination is nonrefundable; exceptions reviewed case by case, subject to applicable law. Labs, imaging, and medications cost extra.
Acute Visits — $85/visit
Focused care for minor illnesses and new symptoms, such as coughs, sore throats, urinary symptoms, and rashes. Includes an evaluation, treatment plan, and prescriptions when appropriate.
For nonmembers: Up to two visits per calendar year. Payment due at booking; medications cost extra.
No lab testing, chronic disease care, ongoing medication management, or follow-up care. These visits do not establish us as your primary care provider.
